Insurance Participation
Westwood Dermatology participates in most major health insurance plans including Medicare, Horizon Blue Cross Blue Shield, Empire Blue Cross Blue Shield, United Health Care, Oxford, Cigna, Aetna and Qualcare. However, please verify at the time that your appointment is made that the doctor you wish to see participates with your particular insurance plan. Not every physician in our office participates in every health plan with which we affiliate.
Out-of-network services are available to all patients. Patients seen out-of-network will be given the appropriate paperwork to submit to their insurance companies. These patients are expected to pay in full at the time of visit.
Cosmetic Procedures: Payment is expected in full at the time services are rendered.
Office Policies
A valid insurance card is required at the time of the office visit. If you do not have a valid insurance card, you will be asked to sign a waiver and the cost of the visit will be your responsibility, payable at the time of service.
HMO/managed care patients requiring a referral from their primary care physician (PCP) must have a VALID REFERRAL before being seen. We are unable to see any patient without a valid referral from your PCP even if an appointment has been made.
All co-payments must be paid at the time of service. This arrangement is part of your contract with your insurance company. We accept MasterCard, Visa, Discover, cash and checks. If you do not have the co-payment at the time of visit, we will charge you a $15 administrative fee in addition to the co-payment.
Westwood Dermatology has a 24 hour cancellation policy. Patients who fail to cancel a scheduled appointment within 24 hours or who do not show up for their scheduled appointment, will be charged a $25 fee. We understand that emergencies do occur and will take that into consideration.
New patients should arrive 10 minutes prior to their scheduled appointment so that a complete chart can be processed in our computer system. All new patients must show photo ID at the time of their appointment.
